To submit a warranty claim, please fill out the form below. After you click “Submit,” a member of our customer service team will get back to you as soon as possible with the next steps.
If you would like to make multiple warranty claims for different products, please submit one form per product.
Additional information such as proof of purchase (receipt) and a photo
of the product damage will be requested by the Customer Service Agent once they
receive this initial request. Please complete the following request form with
your personal information.
- All Cascade helmets and Headgear have a One Year Warranty.
To obtain an RA number you will need the serial number from the Cascade product so please remember to provide. The serial number can be found under the left ear pad.
- All Cascade Eyewear have a Two Year Warranty.
To obtain an RA number you will need the serial number from the Cascade product so please remember to provide. The serial number is located on the inside frame of the product.
- All Maverik Protective-Bags Apparel have a One Month Warranty.
- All Maverik Heads-Shafts-Sticks have a Six Month Warranty
